Refunds

Please talk to Mr. Hoffman first if you must back out of summer camp. We may be able to refund you fully if another scout from our troop can take your spot.

Generally only an 80% refund is possible, and this only if we can notify Council by June 14, 2009.

Need backup parents to drive up to Wente Scout Reservation.

Must be at Wente Scout Reservation by 9:00 am Sunday. (3 ½ hour drive)

Parent availability

Should there be a need for your son to return from camp early (medical, behavior, or other reasons), you must beavailable to come and get him. This also means that you must be able to receive a phone message at any time.

New Medical Form

The National Council of the Boy Scouts of America, and the San Francisco Bay Area Council have issued a new”Annual BSA Health and Medical Record”.

Part A, and Part C are required for attending camp this year.

While the Part B, Medical Exam, will not be required until January 1st, 2010, Troop 612 would like to have you use it for this year’s camp.

Important: National will no longer accept any other medical exam form.. Part B of the “Annual BSA Health and Medical Record” must be used.. You can attach other forms as supplimental information for the Adult Leaders, and care providers in case of an emergency, but these can no longer replace the required medical exam form.

Medical Form Part A

This is required to be updated every year.

General Information

Fill in the Scout’s personal informaion, and emergency notification information.
Make a copy, front and back, of the Scout’s insurance card, and attach this to the medical record.
Make sure that the Scouts’s personal physician’s name and phone number are correct.

Medical History

Fill out the medical history and shot record. You can consult the prior medical forms to transfer dates.

Medications

List all medications used. Be sure to attach a medication form for each medication.

Medical Form Part B

This is required for resident camp, events over 72 hours, and activities which are strenuous and demanding. It is valid for one year after the exam..

Physical Examination

Must be signed by a MD/DO/DC/PA/RNP.
Date must be greater than 7/5/2008 and legible

For this year only, if your Scout has a earlier medical form with a doctors signature greater than 7/5/06, then that can be attached. However, we recommend a current exam for the safety of your Scout. Adults over 40 must use the new Medical Record.

Medical Form Part C

This is required for your Scout to attend camp

Informed Consent

To allow your Scout to participate in all activities, Check the YES box.

To withold permission to pariticipate in one or more specific activities, Check the NO box and specify which activities should not be permitted.

In the event of an emergency, when the emergency contact can not be reached the camp requests your permission to allow the Scout to be treated by a care provider selected by the Adult Leaders, and to allow that Adult Leader to disccuss and consult with these providers

To allow this without restrictions, check the WITHOUT RESERVATIONS box. Otherwise check the WITH SPECIAL CONSIDERATIONS OR RESTRICTIONS box and indicate those on the line below.

Talent Release

This allows the Council and Troop to use your son’s image.

Confirmation

We need your Scout and yourself, as parent/guardian to sign the form in order to permit the Scout to attend BSA activities during the year, including Summer Camp.

Insurance Card

You should attache a copy of your Scout’s Insurance card, or attache it on a separate sheet.

Old Medical Form

If your Scout had a medical exam in the last 36 months prior to the completion of summer camp (exam dated 7/6/06 or later), then this can be used THIS YEAR ONLY, instead of the new Part B.

Top section: (Class 1 part B)

Make sure all information is filled out. The “tetanus toxoid” shot information is mandatory.

Bottom section: (Class 2)

Must be signed by a MD/DO/DC/PA/RNP. Date must be greater than 7/5/2006 and legible. (This section is valid for 36 months.)

You may attach forms issued by Kaiser, or used for other activities so long as they cover the items listed in this section. Make sure the physician’s name, signature and date are legible.

Medication Form

Fill out a separate form for each medication your son will be using. Turn the form in by June 16th to Mr. Hoffman

Medications

Put each medication in a labeled zip-lock bag, and place these in a larger zip-lock bag with your son's name on it. Turn these into Mr. Passinissi on Saturday morning. Mr. Passinissi will ensure that each medication matches a medication form. Retrieve medications from Mr. Passinissi on Sunday afternoon.

Medical Form turn in date

June 16th.

Camp Uniform

Each Scout should bring their troop cap, two troop t-shirts, two Scout shorts, two pair of long Scout socks, and one regular Scout uniform shirt. Mr. Brewer has order forms for the Troop t-shirts. Mrs. Bryson can sell your son a troop hat if you need a replacement. OA members should bring their sash.

What to bring.

Each should should have not only the Scout Essentials but also the Summer Camp items.

How to mark personal items

Scout’s name, troop number. Use a sew on label or permanent marker.

What not to bring

Please follow the normal list of forbidden items that should not be taken on outings.

Personal Food

Please do not allow your son to bring food to camp. The camp has a large number of deer, raccoons and smaller varmints that are very used to the presence of people and have no qualms about searching for food in tents and backpacks.

Spending money

$10 to $25 in ones in a sealed standard sized envelope with your son’s name and the words “Spending Money” on the outside. Turn this into Mr. Hoffman on Sunday of departure. We will not be able to make change. Please do not give your son excessive money. We discourage purchases of excess snacks and trinkets. This same envelope can be used for all other monies sent to camp, just indicate the amount of money being sent and the purpose for which it is being sent. For example:

Camp T-Shirts have been purchased and will be distributed to the Scouts on the last day of camp to wear home. Their cost was included in the camp fee, and we saved $5 per shirt by purchasing them early.

Rising W Corral

NOTE: All riders must be at least 13 years of age and at least 5 foot tall. They must have heavy long pants, such as jeans, and sturdy closed toe shoes or boots.

Breakfast ride: Cost: $20.00. Friday, 7:00 a.m.
Lunch ride: Cost: $20.00. Friday, 11:00 a.m.
Trail ride: Cost: $15.00. Monday, 7:00 p.m., and Friday, 3:30 p.m.
Dinner Ride Cost: $20.00. Thursday, 4:00 p.m.
Outpost ride: Cost: $30.00. Tuesday , 4:00 p.m., and Wednesday, 4:00 p.m.

Other costs

Each Scout should have $7 for lunch on the Sunday return trip.

Some merit badges require the purchase of equipment at camp. The cost is on the attached sheet. Please note that for rifle and shotgun, if the Scout is not already a good shot, the cost can easily be three times the stated amount. These merit badges are not recommended for beginning shooters.

Place the money, in ones, in a sealed standard sized envelope with your son's name and the words "Expense Money" on the envelope. If money is allocated for the Corral Rides a larger bill may be included.

Please list what the money is for on the back of the envelope. Use the sample attached. Money will only be given to your son for the purpose you list.

Turn this into Mr. Hoffman on Sunday of departure.

First year Scouts

First year Scouts should concentrate on the "Eagle Trail". This is a camp program that concentrates on the requirements for Tenderfoot, Second Class, and First Class awards. We seek to focus the new Scouts energy on attaining an achievement within their ability. The camp has rated their Merit Badge classes, and this is attached. Merit Badges with a difficulty rating of "1" are possible to be completed by a first year Scout.

Older Scouts not yet First Class

Completion of the First Class award should be the primary focus of these Scouts. We will discuss merit badges that they may wish to work on with them, but will ensure that they will have every opportunity to complete the First Class award at camp.

Merit Badge work

Each Scout's proposed merit badge work will be reviewed by Mr. Stern to ensure that the Scout is not only able to succeed, but also to have time to have fun. We will try to ensure that each Scout's proposed work will also fit his needs for advancement.

Four moderate merit badges are possible for a motivated second or third year Scout. Hard merit badges should only be attempted by Scouts who are at least third year campers. Many of these require a lot of time, writing of essays, and mental discipline. Please review what your son wishes to work on. Special care should be made to review the material before camp and to complete any pre-camp requirements.

Scouts who do not complete all requirements will be given "partials”. These allow the Scout to complete the merit badge by contacting a merit badge counselor after returning from camp.

Early release of Scouts

If your son must leave camp before the troop leaves on Saturday, the following procedure must be followed:

If an emergency arises, please contact the camp directly to inform them that you will need to pick up your son. The camp will not release any Scout unless these procedures are followed.

Visitors

Parents and adult friends are welcome to visit on Friday afternoon and evening. Please do not come to camp prior to that time. Siblings and other children are not permitted in the camp. Visitors must check in at the Camp Office. Wente Scout Reservation does not have any facilities for visitors. There are many motels in the town of Willits.. Meals can be purchased but advance notice is required

Breakfast $5, Lunch $4; Dinner $6 per person. ($12 for all three meals).

If you plan to visit or wish to purchase meals, please inform Mr. Hoffman.

Special Diets

Please let Mr. Hoffman know as soon as possible if your son has special needs. We need to make sure the camp can accommodate his needs.
